Employee completes Section 1 this form, as required by law, on the first day of employment to show proof of eligibility to work in the US and identity. Employer must examine acceptable documents and record Section 2 by day 3 of employment.
- Meal Period Waiver – First Meal PeriodMay 12, 2026Use this form when an employee voluntarily elects to waive the first meal period on workdays of six (6) hours or less, as permitted under California law, and to document or revoke the waiver.
- Meal Period Waiver – Second Meal PeriodMay 12, 2026Use this form when an employee voluntarily elects to waive the second meal period on workdays of more than ten (10) but no more than twelve (12) hours, provided the first meal period was not waived, and to document or revoke the waiver.
- Notice to Employee (Labor Code Section 2810.5) – Fillable PDF Document (effective November 2023)May 15, 2026Ensure non-exempt employees receive a Notice to Employee (version November 2023) upon hire and within seven calendar days after their wage information changes, or after any of the employment-related information required in the notice changes (unless it is reflected on a timely paystub). Ensure all rates of pay are listed, CA PSL, and workers’ compensation sections are filled out on all Notices to Employee. We recommend adding the Notice to Employee to the new hire checklist.
- Property Return AgreementJanuary 8, 2026Use this agreement to document Company property issued to an employee and acknowledge the employee's responsibility to return all Company property upon separation or whenever requested by the Company.

- Change in Relationship NoticeJuly 6, 2026Use this notice at the time of separation to document an employee's change in employment status and provide information regarding potential unemployment insurance benefits.
- Final Paycheck AcknowledgementJanuary 2, 2026Use this form at the time of separation to document that an employee received their final paycheck and acknowledge the amounts paid. Employers should retain the signed form and a copy of the final paycheck in the employee's personnel file.
- Property Return Reminder LetterApril 13, 2026Use this letter after an employee separates from employment if Company property has not been returned. It reminds the former employee of their obligation to return Company property and requests its prompt return.

Employers subject to CFRA provide this fact sheet to Spanish-speaking employees requesting leave under CFRA to care for their own serious health condition or a family member with a serious health condition, or to bond with a new child (via birth, adoption, or foster care.)
- Form 4504 Spanish Medical CertificationJanuary 1, 2023Use this form to obtain medical certification when a Spanish-speaking employee requests FMLA/CFRA leave due to their own serious health condition or to care for a qualifying family member. If requested, employees generally must return the completed certification within 15 calendar days. Maintain all medical certifications in a confidential medical file, separate from the personnel file.
